Group vs Individual Health Insurance
Group plans cover members under a master policy with employer premium sharing, guaranteed eligibility windows, and no individual underwriting. Individual plans are underwritten (or ACA community-rated) and portable with the person.

What is a pre-existing condition, as the term is typically used in individual health insurance?
A physical or mental condition that existed before the policy's effective date and may be excluded or subjected to a waiting period — A pre-existing condition is a condition for which the insured received treatment, advice, or diagnosis, or which showed symptoms, before the policy's effective date. Individual policies historically applied pre-existing condition exclusions or waiting periods so that applicants could not buy coverage after a condition appeared and immediately claim benefits. Under the Affordable Care Act, pre-existing condition exclusions are prohibited for most individual and group health plans, but the concept remains important for understanding disability income insurance and supplementary limited policies where such limitations may still appear.

Which statement correctly describes a qualified small employer HRA (QSEHRA) or an individual coverage HRA (ICHRA)?
They are employer-funded arrangements that reimburse employees for qualified medical expenses, including premiums for individual health coverage — A QSEHRA and an ICHRA are both employer-funded health reimbursement arrangements: the employer contributes money, and the arrangement reimburses employees for qualified medical expenses — including premiums for individual health insurance coverage purchased on or off the marketplace. The QSEHRA is limited to small employers that do not offer a group plan, while the ICHRA is a broader option for employers of any size. In both, the employer funds the account and the employee owns the underlying individual coverage. Both arrangements must meet IRS requirements, including providing a written notice to employees, but they differ in which employers may use them and in how they interact with the health insurance marketplace.
